Initial Comments:
Based on the findings of an off site follow-up survey completed on May 16, 2023, From The Heart Companion Services, was found to have corrected the deficiency cited under the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries. The deficiency was cited as a result of an unannounced on-site, state re-licensure survey conducted on February 2, 2023.
